Hamstring Tendinopathy


Hamstring tendinopathies are commonly seen at the proximal attachment where the hamstring attaches onto the ischial tuberosity. Typically this condition will develop over time and is associated with an increased level of activity. Often it is observed in individuals who are regularly exercising or participating in activities that require regular loading of the hamstring.


Common signs and symptoms of hamstring tendinopathy include:

  • Tenderness to palpation of the hamstring near its origin at the ischial tuberosity.
  • Tightness in the hamstring muscle belly.
  • Discomfort and or awareness of pulling on a hamstring stretch or active resisted knee flexion.


Diagnosis is typically based on the case history presentation and physical assessment. In some cases the osteopath, GP or sports doctor may refer for further imaging.

Treatment and management typically involves modifications of activities to reduce further aggravation of the injury. Hands on manual therapy such as massage, manipulation and dry needling may also be performed to treat the pelvis and lower limb. In some cases the individual may also take over the counter medication or prescription medication or undergo injection based therapies. Exercise prescription and rehabilitation should also form a part of the management plan so that normal range of motion and strength and conditioning can be restored.
